My hosting provider ~probably~ collects an apache access log.

An IP address can be used to locate a computer down to the town/city, law enforcement can find an address using an IP address.

Here are some ways you can mitigate the chances of someone finding you on the web.
Most secure: https://tails.boum.org/ – An operating system that only runs off a usb/dvd drive. All information created is lost when the computer is powered off. All network traffic is sent through Tor, a three-hop encryption network.
Less Secure: https://www.torproject.org/ – The Tor browser, runs on your computer. All information is sent through the Tor three-hop encryption network.
Least Secure: https://protonvpn.com/ – Proton VPN, you may have heard of VPNs before, no, they’re not magic, and NO, they’re not as good as the two listed above. But for the simple task of making your IP address not be your IP address anymore, they can work. VPNS are large targets for having back doors installed by hackers and governments alike, although they promise they don’t . They are a single-hop encryption network. A single point of failure. (Even fancy ones like Proton VPN Plus with it’s “Secure Core” multi-hop feature is still a single point of failure)
